Description
This plugin was created to help with user manage forms, form submission, and email notifications. Not only does it handle forms it also tracks all submissions right from the dashboard and features a variety of styling / notification options.

Here you may view forms and create new ones! 
Creating a new form has quite a few options. 
Once created you can Add Fields and edit/re-arrage the fields to show up on the form. 
There are a few different fields available with the plugin; even a built in Google Recaptcha. 
You can also modify the form template, text template, and textarea template if your proficient in html. 
Edit the message the users will see once the submission is made. 
Customize emails to your admin and the email template you will see. 
You can also view submission made for this particular form in the dashboard.
Installation
This section describes how to install the plugin and get it working.
- Install the plugin via the Plugin >> Add Plugins on you admin dashboard.
- Activate the plugin through the ‘Plugins’ menu in WordPress
- From there you will see a new option for Flaming Form.
- Create a new form using our easy to use editor.
- Use the email options to configure notifications.
- Add the shortcode to a page.
- As submissions come in you can see them via the Submissions button.

So I used their password reset plugin and decided to give this one a try! It was great to have all the built in options it does. I didn’t have to install other plugins to get Google reCaptcha or saving submissions to the database; it was all built in. Good product, I hope it gets a following!
